     I, , hereby give permission for my son/daughter , to attend Edwards Driving School and promise to pay *** in full by the completion of the driver's education course.  I do give permission to train my son/daughter in all aspects of the driver's education course.  The minimum requirement for this course is thirty (30) hours of classroom instruction and eight (8) hours of behind the wheel training.  I understand that this does not include any of the fees for off-site testing.

Requirement for Off-Site Testing are as follows:

1. Proof that the student is at least 16 years of age

2. Notarized parental consent form

3. Two forms of identification

4. Passport-sized photo

5. Check or money order for *** payable to D.M.V.

6. Check or money order for *** payable to Edwards Driving School.

7. Address verification

ALL ITEMS MUST BE SUBMITTED AT LEAST FIFTEEN (15) DAYS PRIOR TO THE TEST DATE.

Student/Parents/Guardians wishing to terminate their contracts before the start of the classroom cycle, will incur a Fifty Dollars ($50.00) processing fee, to be deducted from their deposit.  Deposits are forfeited if either of the aforementioned parties terminates his contract, while the student has already started classroom.  A monthly service charge of 1.5%, or 18% APR will be added to all overdue accounts and liable for all legal and collection fees.

Classes begin promptly on Monday at 5:00 p.m. to 7:00 p.m. and Thursday at 5:00 p.m. to 7:00 p.m.
