Download and print the Enrollment Form and bring it with you to your first class.

 

The parental consent section is for the parent/guardiuan to fill out if the student is under 18. If you are going to email the Enrollment Form and are aged under 18, a parent/guardian MUST sign the consent section on the first night of class.
